Case discussion: How would you treat this patient? [7 August]

This week we have another case discussion from Dr Slavko Doslo. It is about an elderly man presenting for reasons unrelated to his skin. A full skin check is done and these clinical and dermoscopic images are taken. How do you evaluate them?

This is the pathology result. What is your conclusion and what are the next steps you would take to treat this patient?

Chaos with regression and peripheral grey clods / clues.
Excision with 2mm Margins as initial management.
Orthogonal Lines , Regression, Milky pink structure less area .
Extensional Bx to R/O Lentiginus Melanoma .
Pink brown area, with some linear vessels, peripheral grey dots, grey area, some peppering, some short angulated lines???……excise to rule out MM, LM
dermoscopic grey, one polymorphic vessel and chaos on a loner lesion ? severely dysplastic nevus
I would excise with 5 mm margins
LM is a in situ melanoma. Excision with 5mm margins is appropriate after histological diagnosis.
